The Application Process: A Step-by-Step Guide
Making the decision to move is never an easy one—for any of us. St. John’s understands the magnitude of this decision and are here to walk you through the process, every step of the way. We have put together this step-by-step guide to shed some light on the application process for the different levels of senior services and to let you know what to expect when applying to live at one of our communities. Independent Housing Application Process
Step One: Schedule your visit to identify any housing preferences and ensure our community is a good match for you or your loved one. Our independent living communities are still available for tours following guidelines aimed to keep you and our residents safe. For those looking into our enhanced assisted living, we are not doing tours at this time due to the governor implemented COVID-19 restrictions, but can provide you with virtual options to view our community.
Step Two: Once you are confident that we are the right fit, submit an application. For enhanced assisted living, the application is available for downloading online. Submitting an application is free and all applications are reviewed within 24 hours. You can submit an application electronically by emailing one of our senior housing sales representatives: Dawn Rivera (Brickstone by St. John’s or Enhanced Assisted Living at St. John’s Meadows); Cindy Ruscio or Stephanie Krebbeks (St. John’s Meadows)
Step Three: If approved, your apartment will be updated with a new carpet, a fresh coat of paint, and a professional cleaning prior to move-in. This process typically takes about a month.

Skilled Nursing Care Application Process
Coming from a hospital or another nursing home?
Step One: An application must be requested by the family or at the hospital’s request. Please note that due to COVID-19 restrictions and guidelines, we are not doing facility tours at this time. However, we can provide you with virtual options to view our community.
Step Two: Submit your application for approval. Applications are free to submit.
Step Three: The application and clinical information from the hospital will be reviewed. Once approved, the process will move forward and we will reach out regarding the steps to facilitate the move-in. This process can be completed relatively quickly depending on the level of urgency of your loved ones situation.
Coming from home or assisted living?
Step One: Request an application and a medical release form.
Step Two: Submit the completed application and the medical release form to the Admissions Department.
Step Three: Once we review the application and receive the clinical information from the physician, the process will continue forwards and we will reach out about getting you set up to move in.
Advice for Future Applicants
The best piece of advice we can offer is to plan ahead if possible. Talk to a care manager, physician, or elder attorney ahead of time so you have a plan in place when you or your loved one are in need of assisted living or skilled nursing care. Making the decision to move is never easy, but in our experience, it is much easier to make the decision while you or your loved one are healthy and able. In fact, we often hear, “I wish I would have done this sooner” from residents after they have moved in to our community.
