UR Medicine Geriatrics Group Brings Clinical Excellence to St. John’s Home
St. John’s Home has welcomed Dr. Dallas Nelson as its new Medical Director through a new partnership with UR Medicine Geriatrics Group (URMGG). The transition of medical services to URMGG began September 1 and is part of St. John’s Home’s ongoing efforts to strengthen clinical quality and support continuity of care.
Dr. Nelson has been a familiar face for many people throughout St. John’s for nearly two decades. She has served as a primary care provider for hundreds of St. John’s Meadows and Brickstone by St. John’s residents since 2008.
“I am looking forward to continuing to serve the St. John’s Meadows community as I have been and growing into St. John’s Home as the medical director,” Dr. Nelson said.
Through the affiliation with the UR Medicine Geriatrics Group, residents and families will have access to medical providers’ documentation through MyChart. Dr. Nelson noted that the nursing home’s official record remains PointClickCare. She encourages residents and families to use MyChart to follow their medical providers’ notes only. All communication should be done through the nursing home team. This transition will also not impact residents’ existing insurance plans or coverage.
A key focus for Dr. Nelson and the UR Medicine Geriatrics Group will be providing personalized care based on each resident’s goals and unique needs. The group also aims to reduce unnecessary emergency department transfers and hospital admissions by working closely with St. John’s Home staff to identify and treat conditions early.
“With our close ties to the UR system, we will be able to advocate for the right care, for the right person, at the right time,” Dr. Nelson shared.
Dr. Nelson also looks forward to working alongside the St. John’s Home team to continue improving care for residents. She said the UR Medicine Geriatrics Group is committed to delivering “ever better” care in keeping with the University of Rochester’s Meliora motto.
Residents at St. John’s Home will be supported by physicians Dr. Asma Bawaney, Dr. Christina Rusu, and Dr. Mary Bonnet, who will provide consistent coverage at 150 Highland Avenue.
The partnership between St. John’s and UR Medicine Geriatrics Group is part of a broader effort to strengthen clinical quality at St. John’s Home and position the community to achieve 5-Star performance over the next three years.
