St. John’s Home Announces Transition to TeamHealth Medical Services

Rochester, NY — August 2025 — St. John’s Home has announced a transition in the delivery of medical services for its residents. Effective September 8, 2025, the organization will move from its in-house medical team to a contracted partnership with TeamHealth, a leading provider of post-acute, comprehensive clinical services serving 49 partner communities in the region.

The decision follows a thorough assessment of resident needs and the evolving healthcare landscape. According to St. John’s leadership, the new model will strengthen care delivery by providing:

  • Expanded resources in specialties such as wound care, cardiology, and behavioral health

  • Greater flexibility in medical coverage and scheduling

  • Access to the latest advancements in geriatric medicine

“Preserving resident wellbeing and maintaining continuity of care remain our top priorities throughout this transition,” said Daniel Morphet, Vice President of Skilled Services. “Our current team has provided decades of dedicated service, and we remain deeply grateful for their contributions. This change is designed to build upon their legacy while equipping us with the tools to meet increasingly complex medical needs.”

Residents and families are assured that there will be no gap in services during the transition. TeamHealth practitioners will collaborate with St. John’s nurses and caregivers, provide a consistent on-site presence, and remain available 24/7. All existing treatment plans and advance directives will continue to be honored.
